Bipolar disorder involves distinct episodes of elevated or irritable mood with increased energy, reduced need for sleep, and impulsive decisions, alternating with depressive periods. Because people usually seek help while depressed, the manic or hypomanic side is often missed.

Accurate diagnosis matters: antidepressants used alone can worsen mood instability in bipolar spectrum conditions, so the evaluation carefully reviews lifetime mood history, family history, and past medication responses.
